MPSERS Retirement Documents Needed: The Complete Checklist

The Core Documents

Start gathering these six to twelve months before your target retirement date. Replacements take weeks to arrive, and a missing document can stall your application.

Proof of age (member): ORS requires proof of your age. Preferred proofs include a government-issued birth certificate, valid U.S. passport, valid driver's license or state ID, delayed birth certificate, or qualifying Social Security Administration documentation. If you don't have an accepted preferred proof, ORS accepts two items from its alternate-proof list. If you were born outside the U.S., accepted options also include a valid foreign passport, immigration record, or naturalization record.

Proof of age (spouse/beneficiary): ORS needs proof of age for your pension beneficiary and, when applicable, your spouse or insurance dependents. Use the same acceptable proof list above; a government-issued birth certificate, valid passport, driver's license, or state ID is preferred.

Proof of marriage: ORS requires proof when you name your spouse as pension beneficiary, enroll them in insurance, or document a spousal waiver. A government-issued marriage certificate is accepted. ORS also accepts copies of both valid driver's licenses showing matching addresses plus your most recent IRS Form 1040 showing married filing.

Divorce decree(s): If you were divorced during your MPSERS membership, bring the final judgment of divorce. If the decree requires a pension division, the Eligible Domestic Relations Order (Form R0259C) must be on file with ORS before your retirement effective date.

Military discharge papers (DD-214): Required only if you're purchasing military service credit. The DD-214 must show the character of discharge and dates of active duty.

Social Security benefit estimate: Required only if you're considering an Equated Plan option. Request your projected age-62 benefit from SSA at ssa.gov or your local Social Security office.

The Forms

Retirement Application: The application window opens three months before your retirement effective date; apply online through miAccount during that window. This isn't a paper form — you complete it in the miAccount portal.

Pension Election and Spousal Waiver (Form R0869C): Generated in miAccount during the application process. Required if you're married and selecting anything other than the 100% Survivor Option with your spouse as beneficiary. Must be signed by both you and your spouse in front of a Notary Public.

TDP Payoff Worksheet (Forms R0518C & R0718C): Only needed if you have an active Tax-Deferred Payment agreement. Your employer's payroll office processes the payoff calculation.

Act 88 Application (Form R3571C): Only needed if you're requesting reciprocal service credit from another Michigan public retirement system under the Reciprocal Retirement Act of 1961.

Military Service Credit Application (Form R0081C): Only needed if purchasing military service credit.

Refund Beneficiary Designation (Form R0748X): Names the person who receives any remaining accumulated employee contributions if you die before those contributions are paid out through pension distributions.

Common Processing Delays

The documents most likely to cause delays are the spousal waiver (R0869C) and the EDRO (R0259C). The spousal waiver gets rejected for correction fluid, missing signatures, or date mismatches with the notary acknowledgment. The EDRO requires court approval and ORS review, which can take months.

Everything else is straightforward — but "straightforward" still means you need to order certified copies from vital records offices that process requests on their own timeline, not yours.
