CalSTRS Survivor Benefits: How to Claim Step by Step

There Is No Single Claim Form

One of the most common frustrations for CalSTRS survivors: there is no single downloadable PDF "death claim form" that you can fill out and submit. The process works differently.

You initiate the claim by reporting the death online at CalSTRS.com or by calling 800-228-5453. CalSTRS assigns a caseworker to your file, who reviews the specific account details — coverage type, membership tier, beneficiary designations, and any community property orders — and mails a customized survivor benefit application packet.

The initial quote letter, Form SB401, arrives within 60 calendar days of the death notification. It outlines available benefits and required documents; the caseworker sends the application forms for your claim.

The Four-Phase Timeline

Phase 1: Report the Death (Days 1–15)

Report online or by phone. Provide the member's full legal name, SSN or CalSTRS Client ID, date of death, and your contact details. If reporting online, attach the death certificate during the initial submission — it's a one-time upload window.

CalSTRS freezes the account and begins recalling any pension deposits issued after the month of death.

Phase 2: Wait for the Quote Letter (Days 16–60)

The assigned caseworker reviews the account and mails Form SB401 — the Initial Quote Letter. This document tells you exactly what benefits are available, what payout options exist, and what documents you need to submit.

Phase 3: Gather Documents and Submit (Days 61–90)

Assemble everything the caseworker requests:

  • Certified death certificate with the county vital statistics office seal
  • Proof of birthdate for option beneficiaries (birth certificate, passport, or driver's license)
  • Marriage certificate or domestic partnership registration for spousal claims
  • Completed claim forms from the caseworker's application packet

Submit the complete package to CalSTRS. Incomplete submissions are the number-one cause of processing delays — a single missing document resets the clock.

Phase 4: Processing and Payment (Days 91–120+)

Once CalSTRS receives and verifies all documents:

  • Monthly option continuance allowances process within 60 calendar days
  • One-time death benefit lump sums process within 120 calendar days

If a community property claim exists (divorce or legal separation during the member's career), add another 30 days for the Office of General Counsel review.

The Steps CalSTRS Does Not Handle

CalSTRS administers pension benefits. It does not:

  • Notify the school district about health insurance continuation (you have 60 days)
  • File your Social Security survivor benefit claim (call SSA at 800-772-1213)
  • Handle 403(b) or 457 plan death benefits (contact the plan vendor)
  • Advise you on tax treatment of lump-sum payouts (consult a CPA)

These parallel tracks run on their own deadlines. Missing the 60-day health insurance notification window, for example, can cost a surviving spouse their coverage entirely.
