Start Gathering Documents Before the Application Packet Arrives
CalSTRS assigns a caseworker after a death is reported and mails an initial quote letter (Form SB401) within 60 days. The letter outlines available benefits and required documents. But the documents you'll need are predictable — and obtaining them takes time. Start now.
Required Documents
Certified Death Certificate
A certified copy with the official seal from the county vital statistics office. An informational copy (marked "not a valid document") will not be accepted. Hospital or funeral home certificates are not sufficient.
Order at least four certified copies — you'll need them for CalSTRS, the school district, Social Security, and potentially the bank or probate court. Depending on the county, certified copies cost $20–25 each and take 2–4 weeks to arrive if ordered by mail.
Proof of Birthdate (Option Beneficiaries)
If you're claiming a monthly option continuance, CalSTRS requires proof of your date of birth. Acceptable documents:
- Certified birth certificate
- Valid U.S. passport (identification page)
- State-issued driver's license
- Military ID
This establishes the actuarial basis for your lifetime benefit calculation.
Proof of Relationship
Surviving spouse: A certified marriage certificate from the county clerk's office where the marriage was recorded. If the original is lost, request a certified replacement — this can take 4–6 weeks depending on the jurisdiction.
Registered domestic partner: A California Secretary of State Domestic Partnership Registration or a certified copy of the original declaration.
Both: If the surviving spouse or partner cannot be located, is incapacitated, or is legally separated, the member may have filed Form MS1125A (Justification for Non-Signature of Spouse or Partner) to relieve the signature requirement. The caseworker will verify this against the file.
Recipient Designation Form (MS0002)
This form, filed by the member before death, determines who receives the one-time death payment and DBS/Cash Balance lump-sum distributions. You don't need to submit it — CalSTRS has it on file. But knowing whether one was filed (and who was named) is important.
If the member never filed an MS0002, or if all named recipients predeceased the member, the one-time death payment goes to the estate. That triggers probate proceedings or a California Small Estate Affidavit (Probate Code § 13100) if total personal property falls below the threshold.
Claim Forms From the Caseworker
The application packet from the assigned caseworker includes specific claim forms tailored to the account. These must be completed, signed by all required parties, and returned to CalSTRS.

Beyond CalSTRS, you'll need documentation for parallel claims:
- School district: Death certificate and proof of relationship to preserve health insurance continuation within the 60-day window
- Social Security: Death certificate and marriage certificate to file for survivor benefits (call 800-772-1213 — survivor claims cannot be filed online)
- 403(b)/457 plan vendors: Death certificate and beneficiary claim forms specific to each vendor
