CalPERS Application for Survivor Benefits: Forms, Documents, and Steps
How the Application Package Arrives
CalPERS does not have a single downloadable PDF form for survivor benefits. After you report a death — through the myCalPERS portal, by phone at 888-225-7377, or at a regional office — CalPERS generates a customized application package tailored to the deceased member's specific circumstances. The forms you receive depend on whether the member was active or retired, their tier (Classic or PEPRA), and what their employer contracted for.
Active member deaths get the pre-retirement package (PUB 55/56 guidelines). Retired member deaths get the post-retirement package (PUB 60/62 series). CalPERS sends the customized package after the death is reported.
What the Package Contains
The application form itself requires detailed information beyond what you might expect:
Full family disclosure. You must list every surviving family member — current and former spouses, registered domestic partners, all children (including from prior relationships), parents, and siblings — regardless of whether they expect to receive anything. CalPERS uses this to verify statutory beneficiary rights under the California Government Code. Omitting someone, even someone you believe has no claim, can stall processing.
Benefit election forms. If the member was active and eligible for retirement at death, the surviving spouse or partner must choose between monthly allowance options (Pre-Retirement Option 2W, 1957 Survivor Benefit) and a lump-sum payout (Basic Death Benefit). This is a consequential decision — choosing a lump sum over a monthly allowance permanently ends eligibility for CalPERS health insurance.
Tax withholding elections. Federal and state tax forms (myCalPERS-1289 or W-4P) for monthly allowances, or distribution election forms for lump sums.
Direct deposit authorization (Form CalPERS-1288) for ongoing monthly payments.
Required Supporting Documents
Gather the certified supporting documents that apply to your claim, including:
- Death certificate that explicitly states both the cause and manner of death. A certificate that says "pending investigation" or omits the cause will delay the claim. Order multiple certified copies from the county vital statistics office.
- Marriage certificate or domestic partnership registration from the California Secretary of State.
- Birth certificates for any minor or dependent children who may be eligible.
- Divorce decrees for every prior marriage of both the deceased member and the surviving spouse. If the member was married three times, CalPERS needs dissolution documents for the first two — even if those marriages ended decades ago.
The document assembly is where most claims get stuck. Tracking down a 1987 divorce decree from another state while grieving is the kind of bottleneck CalPERS cannot help with.

The 45-Day Processing Window
CalPERS targets 45 calendar days from the date they receive a complete application with all required certified documents. The clock does not start when you mail the forms — it starts when CalPERS has everything.
Spouses claiming automatic Survivor Continuance face a separate deadline: return the application within 60 days to prevent monthly allowance suspensions.
Common Mistakes That Cause Delays
Incomplete family history (leaving off a child from a prior relationship or former partner). Death certificates missing the cause or manner of death. Submitting ordinary photocopies instead of required certified records. Forgetting prior marriage dissolution records. Missing items can delay processing; the 45-day period begins when CalPERS receives a complete application and all required supporting documents.
