Start Here: Report the Death to STRS Ohio

Do this within days, not weeks. Reporting the death triggers three things: STRS Ohio freezes the deceased member's account to prevent unauthorized changes, halts automated monthly direct deposits to prevent overpayments, and begins assembling the customized Survivor Benefit Packet tailored to the deceased member's specific account.

The Survivor Benefit Packet is your roadmap. It contains forms specific to the deceased member's plan type, benefit elections, and designated beneficiaries. Use its instructions or confirm with STRS Ohio before substituting a generic website form.

Documents You Need to Gather

Start collecting these before the Survivor Benefit Packet arrives. Most families need 5 to 10 certified copies of the death certificate because multiple agencies require originals.

Required for all claims:

  • Certified death certificate listing cause and manner of death (STRS Ohio requires certified copies, not photocopies or informational copies)
  • Claimant's government-issued photo ID
  • Claimant's Social Security number
  • Direct deposit information (bank routing and account number for benefit payments)

Required depending on relationship:

  • Marriage certificate (surviving spouse claims)
  • Birth certificates (child beneficiary claims)
  • Court-appointed guardianship order (for minor children or incapacitated adults)

  • Letters testamentary or letters of administration (estate executor claims)
  • Court-appointed guardianship documentation for an incapacitated beneficiary, as required by STRS Ohio
  • Form W-4P (federal tax withholding; follow the Survivor Benefit Packet's instructions)

Death Certificate Requirements

STRS Ohio requires certified copies of the death certificate. Certified means the document is issued by the vital records office of the county or state where the death occurred, bears an official seal or stamp, and lists the registrar's signature.

Hospital-issued certificates, photocopies, or "informational" copies stamped "not valid for legal purposes" are not accepted.

Order at least 5 certified copies. You'll need them for STRS Ohio, the Social Security Administration, private life insurance carriers, banks, and potentially probate court. Confirm the current fee with the county vital records office or the Ohio Department of Health before ordering.

The Claim Process Step by Step

The claim workflow from death notification to first payment can take about 71–90 days or longer, with complex cases taking additional time.

Active Member Death vs Retiree Death

The core notification and document-gathering steps are similar, but the packet and benefit determination differ:

Active member death: If the member met the applicable service-credit and timing rules, qualified survivors choose among available monthly annuity options (dependent-based, service-based, or retirement-based formulas) or a lump-sum account refund. The refund election becomes irrevocable once the check is issued. If the member was actively teaching, also notify the school district HR department to halt payroll deductions and finalize leave payouts.

Retiree death: No choice — the Plan of Payment selected at retirement governs what continues. The claim packet confirms which plan was elected and what the survivor receives. The $1,000 statutory death benefit is automatically payable to the designated death benefit beneficiary.
