NC Retiree Health Insurance: State Health Plan Eligibility by Hire Date

The most common assumption TSERS members carry into retirement planning is that 30 years of state service guarantees free health insurance. It doesn't. The State Health Plan ties retiree premium subsidies to your initial hire date and qualifying service — not your age or retirement date.

The Three Hire-Date Tiers

North Carolina statute divides TSERS retirees into three tiers based on when they were first hired into a covered position:

Tier A — Hired before October 1, 2006: You qualify for fully state-paid (non-contributory) individual retiree coverage with five years of membership service. This is the most generous tier, and it's closed to anyone hired after that date.

Tier B — Hired October 1, 2006 through December 31, 2020: Your subsidy scales with qualifying service (sick leave does not count toward these thresholds):

  • 20+ years → fully state-paid individual coverage
  • 10 to 19 years → 50% state-paid (you pay the other half)
  • 5 to 9 years → fully retiree-paid (you pay 100% of the individual premium)

Tier C — Hired on or after January 1, 2021: You are ineligible for State Health Plan retiree coverage. The General Assembly eliminated retiree health benefits entirely for this cohort, regardless of how many years you serve.

Dependent Coverage Is Always Retiree-Paid

Across all three tiers, adding a spouse or dependent to your retiree health plan is 100% your cost. The state subsidy covers only the retiree's individual premium. If you're budgeting for retirement and your spouse will be on your plan, this premium is a line item you need to account for before you file Form 6E.

Plan Options for Non-Medicare Retirees

If you retire before age 65 and aren't Medicare-eligible, you'll choose from:

  • Standard PPO Plan (Aetna) — the default; in-network and out-of-network coverage
  • Plus PPO Plan (Aetna) — higher monthly premium, lower copayments and deductibles

You're automatically placed in whichever plan you held as an active employee unless you actively change it through eBenefits within 30 days of your retirement date.

The Medicare Transition at 65

When you turn 65 or become Medicare-eligible through disability, you must enroll in Medicare Parts A and B through the Social Security Administration. The State Health Plan then moves you into one of three options:

  • Humana Group Medicare Advantage PPO Base Plan — $0 monthly premium for retirees qualifying for full state subsidy; Medicare is the primary payer
  • Humana Group Medicare Advantage PPO Enhanced Plan — a buy-up option with lower prescription copays and expanded benefits
  • 70/30 PPO Plan (Aetna) — Medicare pays primary, Aetna pays secondary; requires a monthly premium

One critical trap: if you enroll in any individual Medicare Advantage or Part D plan outside the State Health Plan, you're automatically dropped from the Humana group plan and shifted to the secondary 70/30 plan. That switch typically increases your out-of-pocket costs significantly.

Why Your Service Credit Math Matters

For Tier B members (hired October 2006–December 2020), the 20-year service threshold is the difference between free individual coverage and paying half the premium for life. If you're sitting at 18 or 19 years of creditable service, a service credit purchase — withdrawn service, military time, or out-of-state public service — could push you across that line. But all purchases must be paid in full before your effective retirement date.

Unused sick leave increases the service used in the pension formula, but it does not count toward the State Health Plan's hire-date service thresholds. Check your membership service and eligible service purchases when determining whether you meet a coverage tier.
