How to Report the Death of a NYS Retiree to NYSLRS

Reporting a retiree's death to NYSLRS should happen within the first few days. The system's reclamation process starts immediately once the death is on file, and delays create complications with direct deposits that are harder to untangle later.

Two Ways to Report

By phone: Call NYSLRS Survivor Services at 1-866-805-0990 (or 518-474-7736 if you are in the Albany area). You will need the retiree's full name, NYSLRS ID or Social Security number, date of death, and your contact information as the immediate family member or beneficiary.

Online: NYSLRS has a death reporting form on the Office of the State Comptroller website. It collects the same information and generates a case file. Either method triggers the same internal process.

The formal notification is recorded on Form RS6082 (Notification of Death), though families can submit the report without filling out that form themselves — the phone call or online submission accomplishes the same thing. No notarization is required for this initial notification.

What Happens to Direct Deposits

This is the part that catches families off guard. NYSLRS pension payments are legally payable only through the month of the retiree's death. Once the death is reported, NYSLRS instructs the bank to reverse any direct deposits made for months after the month of death.

If the retiree died on September 15, the September payment is valid. But any deposit for October or a later month will be automatically reclaimed from the bank account. If the funds have already been withdrawn or spent, the family faces a recovery process that can involve repayment demands.

The practical lesson: do not spend pension deposits for months after the month of death until you have confirmed with NYSLRS which payments are legitimate.

The Death Certificate Requirement

After the initial notification, NYSLRS needs an original certified death certificate bearing a raised seal. This is the document that formally opens the benefit determination process. Standard photocopies will not be accepted.

Order multiple certified copies from the funeral director or local registrar — you will need them for NYSLRS, the Employee Benefits Division (for NYSHIP health coverage), Social Security, and potentially banks and insurance companies.
