FRS Form FR-11 Instructions: How to Complete Your Retirement Application
What Form FR-11 Is
Form FR-11 — Application for Service Retirement — is the primary document that initiates your FRS Pension Plan retirement. Filing it tells the Division of Retirement you intend to retire and establishes your effective retirement date. Without a completed FR-11, you're not retiring under the Pension Plan, regardless of how many years you've worked.

When to File
The Division accepts Form FR-11 up to six months before your target retirement date. The earliest you should file is six months out. To preserve the first-day-of-the-following-month effective date, the Division must receive the form within 30 days after your employment termination date — if it receives the form later, your effective date shifts forward, costing you months of pension payments.
Filing early doesn't accelerate your retirement. If the Division receives FR-11 within 30 days after termination, the effective date is the first day of the following month; if it receives the form later, the effective date is the first day of the month following receipt.
Section by Section
Section 1 — Member Information. Your name, Social Security number, date of birth, and mailing address. Double-check the SSN against your FRS Online account; a single-digit error sends your application to the wrong record.
Section 2 — Retirement Election. Your requested effective retirement date. This should be the first day of the month following your last day on payroll. For example, if your last day is May 15, your effective retirement date is June 1.
Section 3 — Employer Certification. This section is completed by your employer's authorized personnel officer, not by you. They certify your termination date, last day physically on payroll, and gross compensation for the final months — including any lump-sum annual leave payout. The employer must sign and submit this section.
If your employer drags their feet on Section 3, follow up aggressively. The Division needs that employer certification to finalize your benefit calculation and issue payment.
Member Signature. Your signature must be notarized. This is the single most common reason applications are returned: the member signed but forgot to have it witnessed by a notary. Many bank branches, UPS stores, and courthouse offices offer notary services.

Forms to Submit Alongside FR-11
Form FR-11 rarely goes alone. Expect to include:
- Form FRS-11o — Option Selection. Your choice of Option 1, 2, 3, or 4. Notarized member signature required.
- Form SA-1 — Spousal Acknowledgment. Required if you're married and selecting Option 1 or Option 2. Both member and spouse signatures must be notarized.
- Birth date documentation — Birth certificate or REAL ID driver's license for you, and for your joint annuitant if selecting Option 3 or Option 4.
- Marriage certificate — If naming your spouse as joint annuitant.
- Form W-4P — Federal tax withholding elections for your pension payments.
Common Errors That Delay Processing
Missing notarization. Every signature field on FR-11, FRS-11o, and SA-1 that says "notarized" means notarized. Unsigned or unnotarized forms come back.
Incomplete employer certification. If Section 3 lacks the personnel officer's signature, a termination date, or final salary reporting, the Division returns it.
Wrong effective date format. The effective date must be the first day of a calendar month. "May 15" is not a valid effective retirement date; "June 1" is.
Unsubmitted supporting documents. Birth date proof and marriage certificates must accompany the application. Submitting them separately often means they get separated from your file and cause processing delays.
