FRS Death Notification Contacts: Phone Numbers, Online Portal, and Who to Call

Reporting a death to the right FRS entity — and doing it quickly — prevents overpayment clawbacks, starts the claims clock, and locks the account against unauthorized changes. The problem is that FRS isn't a single office. Depending on which plan the deceased member was in, you may need to contact two or three different organizations.

Pension Plan, DROP, and Retired Members: Division of Retirement

If the deceased was an active Pension Plan member, a DROP participant, or a Pension Plan retiree, your first call goes to the Division of Retirement within the Florida Department of Management Services.

When you call, have the member's Social Security number and date of death ready. The representative will confirm the account type, stop future monthly deposits (for retirees), and mail or direct you to the correct claim forms — typically Form FST-11b for monthly survivor benefits or Form FST-11g for a DROP balance or contribution refund claim.

For retirees, urgency matters. Monthly pension deposits stop upon notification, but if a deposit has already been scheduled for the month after death, the Division will reverse it through the banking system. Reporting sooner means fewer deposits to recover.

Investment Plan Members: Alight Solutions and the SBA

If the deceased was in the FRS Investment Plan — either as an active employee or a retiree drawing down their account — the administering body is different. The Investment Plan is a defined contribution plan managed by the State Board of Administration (SBA), with day-to-day recordkeeping handled by Alight Solutions.

Alight Solutions will send you the Investment Plan Death Benefit Claim Form. Non-spouse beneficiaries have strict distribution timelines — you must elect a payout method within one year of the member's death, or the account defaults to a full liquidation within five years.

The Deceased's Employer HR Department

Regardless of whether the member was in the Pension Plan or Investment Plan, also contact the employer's human resources department — the school district, county government, state agency, or other public employer. The employer handles:

  • Final payroll processing and any unpaid leave or sick-leave payouts
  • Employer-sponsored group life insurance claims (separate from the FRS pension or investment account)
  • Group health insurance continuation options under the State Group Insurance Program
  • Certification of employment details that the Division of Retirement may request during claims processing

The employer's HR office is also the entity that formally certifies the member's death to the Division of Retirement on the employer side.

If you don't know whether the deceased was in the Pension Plan or the Investment Plan, call the MyFRS Financial Guidance Line at 1-866-446-9377. The counselors there can look up the member's account by Social Security number and tell you which plan they were enrolled in — or whether they had accounts in both (members who executed a "Second Election" may have split histories). From there, they'll direct you to the correct administering body for your claim.

You can also check the deceased's bank statements for direct deposit origins, annual FRS benefit statements, or pay stubs — Pension Plan deposits come from the Division of Retirement in Tallahassee, while Investment Plan distributions come from Alight Solutions in Orlando.
