TRS Texas Beneficiary Documents Needed
Gather These Before the Claim Packet Arrives
TRS sends a custom claim packet after the death is reported. The expected sequence places retiree packets around Days 11–20 and active-member packets around Days 16–25, after employer salary and service certification. But you do not need to wait for the packet to start collecting documents. Every item below takes time to obtain, and having them ready when the forms arrive can shave weeks off your processing timeline.
Certified Death Certificate with Cause of Death
This is the most critical document — and the one most often submitted incorrectly.
TRS requires a certified copy (not a photocopy) that explicitly states the cause of death. Many county vital records offices initially issue short-form certificates or certificates marked "pending" while autopsy or toxicology results are finalized.
Order eight to ten certified copies from the county clerk or vital records office. You will need them for TRS, Social Security, the school district, each bank and financial institution, and any insurance companies. Most Texas counties charge $20–$25 per certified copy.
If the cause of death is delayed, ask the funeral director about timelines for the amended certificate. TRS will accept the claim paperwork before the certificate arrives, but payment will not be issued until TRS receives a certificate with the cause of death listed.
Marriage Certificate
Required if you are claiming as a surviving spouse. TRS needs an official certified copy — not the decorative certificate from the ceremony. If you were married in Texas, request a copy from the county clerk where the license was issued. Out-of-state marriages require a certified copy from that state's vital records office.

Social Security Cards
TRS requires Social Security numbers for both the deceased member and the claiming beneficiary. Having the physical cards (or replacement documentation from SSA) speeds processing. If the cards are lost, you can request replacements through SSA, though this adds a few weeks.
Government-Issued Photo ID
A current driver's license, state ID, or passport for the person filing the claim. TRS uses this to verify the claimant's identity against the beneficiary designation on file.
IRS Form W-4P
If you will receive ongoing monthly survivor annuity payments, TRS requires a completed W-4P (Withholding Certificate for Periodic Pension or Annuity Payments) to set your federal income tax withholding rate. Download it from irs.gov before the claim packet arrives. Getting the withholding right upfront avoids a surprise tax bill in April.
TRS Form 278 (Direct Deposit Request)
For electronic deposit of lump-sum payments or monthly annuities. The form requires your bank's routing number and account number. TRS includes this form in the claim packet, but you can have your banking details ready.
Letters Testamentary or Letters of Administration
These are only required if TRS benefits pass to the deceased member's estate — either because "My Estate" was named as beneficiary or because no valid Form TRS 15 is on file and the statutory default order reaches the estate tier.
Letters Testamentary come from probate court and name the executor authorized to act on behalf of the estate. If the estate's total assets fall below statutory limits, a Small Estate Affidavit under Texas Estates Code Chapter 205 may be accepted instead.
If a named beneficiary is alive and properly designated, probate documents are not needed. TRS death benefits pass directly to designated beneficiaries outside of probate.
Documents You Might Not Expect to Need
Divorce decree and Domestic Relations Order: If a DRO partitions the TRS account, TRS must review it before releasing any payment. Even if you are the current spouse, a prior DRO may award a portion to a former spouse.
Court guardianship order: If the beneficiary is a minor, TRS pays benefits to the surviving parent or court-appointed guardian. Without documentation of guardianship or parental authority, payment stalls.
Power of Attorney or court appointment: If the beneficiary is incapacitated, someone must be legally authorized to file the claim on their behalf. TRS accepts a valid durable power of attorney or court-appointed guardianship.
